PRP can be helpful for some tendon, ligament, arthritis, and sports problems. It is not the right answer for every painful joint, so I start by making sure we know what we are treating.

Before you decide

Patients usually ask because they want something more biologic than a steroid injection, or because they are trying to avoid repeated injections or surgery.

We review the pain source, imaging, prior therapy, prior injections, arthritis level, and whether PRP is aimed at a tendon, ligament, joint, or another structure.

PRP is usually not an instant numbing shot. We talk about cost, activity changes, timing, follow-up, and what would make us change direction.
How I think through the recommendation
Patients should know what would make us wait, what would make treatment reasonable, what might require more testing, and what changes recovery planning.

Injection decision guide
BMAC, PRP, gel, steroid injections, braces, therapy, arthroscopy, and replacement are not interchangeable. I explain the options after we know the target and the severity of the problem.
Think about 01
Tendon pain, mild arthritis, bone-on-bone arthritis, meniscus injury, and referred pain all need different conversations.

Cost, insurance, timing, aftercare, and expected response are part of the decision, especially for PRP and BMAC.

I do not promise cartilage regrowth. If an injection is unlikely to help, I want patients to hear that clearly.

Older patients and caregivers
Safety, medications, stairs, rides, hearing, memory, home help, and caregiver questions can change the recommendation and the recovery plan.

Bring the person who helps with medications, rides, meals, stairs, wound checks, or remembering instructions. A second set of ears helps.

Think about loose rugs, bathroom setup, stairs, lighting, pets, walking aids, shower safety, and where you will sleep the first few nights.

Call for worsening wound redness, drainage, fever, severe swelling, a fall, medication confusion, or symptoms that do not match your instructions.

Chest pain, shortness of breath, fainting, stroke-like symptoms, major deformity, or dangerous symptoms need emergency care.

Make the visit count
Imaging helps, but the story matters too: what changed, what you tried, what failed, and what life you want back. This is especially important for revision opinions, prior surgery, and neighbor island travel.
Recovery preview
Orthobiologic aftercare should explain activity changes, response tracking, realistic expectations, and follow-up.
Patient questions
Coverage varies and is often limited. Patients should confirm cost and coverage before treatment.
Response varies. PRP is not usually an instant numbing injection, so timing and expectations should be reviewed.
It depends on the tear, arthritis, mechanical symptoms, and goals. Some meniscus problems need a different plan.
